Medicare 101

New to Medicare? Start here — a plain-language overview of everything you need to know.

Part A – Hospital Insurance

Covers inpatient hospital stays, skilled nursing facility care, hospice care, and some home health services. Most people do not pay a premium for Part A if they or their spouse paid Medicare taxes long enough.

Part B – Medical Insurance

Covers doctor visits, outpatient care, preventive services, and medical equipment. Part B requires a monthly premium, and most people enroll when they first become eligible to avoid a late penalty.

Part C – Medicare Advantage

An alternative to Original Medicare offered by private insurers approved by Medicare. Bundles Parts A and B (and usually Part D) into one plan, often with extra benefits like vision, dental, and hearing.

Part D – Prescription Drug Coverage

Adds prescription drug coverage to Original Medicare (Parts A and B). Plans are offered by private companies and vary in cost and drugs covered. Enrolling late may result in a permanent penalty.

When Do I Enroll?

Initial Enrollment Period

7-month window starting 3 months before your 65th birthday month.

General Enrollment Period

January 1 – March 31 each year if you missed your initial window.

Special Enrollment Period

Available if you're covered by employer insurance when you turn 65.

Premium

The amount you pay each month for your insurance coverage — whether or not you use any medical services.

D

Deductible

The amount you pay out-of-pocket for covered services before your insurance plan starts paying. Once met, your plan shares the cost.

C

Copay

A fixed dollar amount you pay for a covered service — like $20 for a doctor visit. The plan pays the rest.

C

Coinsurance

Your share of costs after you've met your deductible, expressed as a percentage. For example, you pay 20% and Medicare pays 80%.

O

Out-of-Pocket Maximum

The most you'll ever have to pay in a plan year. Once you reach this limit, your plan covers 100% of covered services.

N

Network

The group of doctors, hospitals, and providers that have an agreement with your insurance plan. Using in-network providers usually costs less.

F

Formulary

The list of prescription drugs covered by a Part D or Medicare Advantage plan. Drugs on the list are covered; those off the list may not be.

B

Benefit Period

For Medicare Part A, this is the span of time for measuring your use of hospital or skilled nursing facility services, starting the day you're admitted.

C

Coordination of Benefits

When you have more than one insurance plan, this process determines which plan pays first and how much the second plan covers after that.

Creditable Coverage

Health or drug coverage you have that is at least as good as Medicare's. Having creditable coverage before enrolling in Medicare can help you avoid late-enrollment penalties.
